I hereby entrust and instruct Bank of China (Hong Kong) Limited ("BOCHK") to apply for making an appointment to open an account /mortgage enquiries referral with BOC mainland or Macau branch or SEA entities on my behalf. I understand that BOCHK only submits the above referral form to BOC mainland or Macau branch or SEA entities on my behalf. The appointment /referral application is subject to the approval of BOC mainland or Macau branch or SEA entities

I confirm that I provide my contact and other personal information set out in the above referral form to BOC mainland or Macau branch or SEA entities on my own initiative. I further confirm that neither BOCHK nor BOC mainland or Macau branch or SEA entities markets the services or products of BOC mainland or Macau branch or SEA entities to me.

本人明白：
(a) 上述轉介表格内容並不構成中國銀行内地/澳門分行或東南亞機構或中銀香港代表其提供或招攬服務或產品。
(b) 中銀香港並非中國銀行内地/澳門分行或東南亞機構的代理人，且無權代表其作出任何陳述或承諾。

I authorize BOCHK, as my agent, to submit the above referral form with my contact and other personal information to BOC mainland or Macau branch or SEA entities, and to receive the information and related documents (if applicable) sent by the BOC mainland or Macau branch or SEA entities to me in relation to the above referral form.

I understand that:

(a) Nothing in the above referral form amounts to an offer or solicitation of services or products by BOC mainland or Macau branch or SEA entities, or by BOCHK on their behalf.

(b) BOCHK is not the agent of BOC mainland or Macau branch or SEA entities and has no authority to make any representation or commitment on their behalf
